Register for Athletic Teams (Final Forms)
The East Ramapo CSD Athletic Department has partnered with Final Forms, a new data management service, for Athletics. Final Forms allows you to complete and sign athletic participation forms for your students. Final Forms saves the data from season-to-season and year-to-year, so you will never need to enter the same information twice! Final Forms also pre-populates information wherever possible, for each of your students, saving you time. You will be required to sign your forms only once per year and after any update.

Spring Sports 2025
Any student-athlete interested in playing a spring sport needs to register at Final Forms and follow the prompts to create your account and register for spring athletics. Parents/guardians of all students planning to play sports must create accounts in Final Forms and register your student(s).
